Regardless of the material (whether synthetic or organic) the effectiveness and duration of any filler is highly dependent on the
amount of activity in the region where it is injected. It is known that high intensity activities such as vigorous exercise can

Over time, the skin on the middle of the face gravitates down changing facial contour from heart shape to square. Role of Dermal Fillers
Dermal fillers are often confused with Botox, injectable muscle relaxant used to smooth out lines and wrinkles. Dermal fillers are injectable implants which integrate with your own tissue and help to smooth skin and wrinkles but more importantly create like a “scaffold” supporting the skin.

Dermal fillers are used for skin rejuvenation, hydration, correction of fine lines and wrinkles, treatment of age-related loss of volume, enhancement and beautification in order to restore facial balance and harmony.

It is based on the same mathematical approach Leonadro Da Vinci used to create beauty and harmony in his works on Mona Lisa. These proportions are based on the Golden Ratio (Phi), which has been used to create beauty by artists and architects. The Golden Ratio is a mathematical ratio 1.618:1. Phi is considered the number 1.618. This ratio is found universally in all beautiful things in nature, including the beautiful human face. Types Of Dermal Filler
Dermal fillers most used for aesthetic reasons are temporary fillers which become degraded over time. They are made of naturally occurring hyaluronic acid, minerals - calcium hydroxyapatite and synthetic substances - polycaprolactone, poly-L-Lactic acid which have been used in medicine and dentistry for years and have very good safety record.
There are several categories of dermal fillers which are generally administered in the same way.
Hyaluronic acid
Hyaluronic acid (HA) is naturally found in skin and cartilage. It is like a sugar molecule which absorbs water and is responsible for retaining moisture in the tissues and elasticity. Hyaluronic acid for cosmetic purpose is made using bacterial fermentation. The HA in its raw form (non-cross linked) can breakdown easily. Cross-linking HA molecules makes the HA more durable, firmer, and longer lasting. Most of the injectable hyaluronic acid dermal fillers are made using cross linking technology and differ in terms of content of hyaluronic acid impacting on their use for hydration, filling or lifting capacity.

There are many brands of hyaluronic acid dermal fillers, each of them consists of products with different characteristics and quantity of hyaluronic acid designed for treating different parts of the face. Each dermal filler is designed for a specific purpose such as to treat fine lines or deeper folds, restore facial volume and lifting effect or to reshape and redefine facial features line nose of lips.
Dermal Filler Outcomes
The outcome depends on the goals you had in mind before your treatment and the products your practitioner selects to help you achieve these goals. This is something we discuss in depth at the consultation stage of the process. Common outcomes from your treatment can be split into two categories;

Potential Side Effects
Potential side effects associated with Dermal Filler include temporary injection-site reactions such as:

Discolouration
These reactions may occur immediately or may be delayed. Most adverse reactions are transient (bruises, swelling, tenderness) and resolve within a week or two. Rare adverse reactions such as an infection, hypersensitivity or delayed granuloma formation may occur with a delay and require treatment. If you develop intense pain, skin discoloration or visual disturbances after dermal filler injections get in touch as soon as possible as to find out if it will require treatment.

MD code technique
Injections are carried out with sterile needle or a cannula using special injections techniques like MD codes described by Mauricio De Mayo a Brazilian plastic surgeon and a global trainer. The procedure is generally pain free although some minimal discomfort may occur.

Dermal Fillers - Step by Step Guide
At Eternal Clinic, we like to make sure our clients know exactly what to expect before embarking on any aesthetic treatment. Pre Treatment Consultation
Before embarking on dermal filler treatment there is a lot to consider such as:

It can be difficult to know what dermal filler to choose from. We will provide you with information and guide you through the process. You do not have to make decision straight away. It is important that you have all the information that you need and are ready to take it forward. Treatment can be carried out over a period. Our motto is “Less is more” when it comes to dermal fillers.
Consultation
During the consultation, we will look at your face whilst it’s still (static) and moving (dynamic) to make sure they’re able to give you a result you’ll be happy with and take pictures to document treated condition before and after treatment. We will ask you about your medical history, medication and allergies.
Before Treatment
Avoid aspirin and other non-steroidal anti-inflammatory medicines to reduce bruising.
Anaesthesia
Most dermal fillers contain anaesthetic lidocaine which helps to minimize pain from the injection. You can have numbing cream applied 30 minutes before treatment to assure maximum comfort.
Treatment
Your injector will use a pre-filled sterile syringe and a thin needle (or blunt cannula) to inject a precise amount of filler strategically in or beneath the skin. The depth of the injection will depend on the type of dermal filler used and desired effect. They might gently massage the area after the injection to help spread out the filler for a more natural look. The entire procedure can take anywhere from 15 minutes to an hour, depending on what you’re having done.
After Treatment
Your practitioner will provide you with post-treatment recommendations and an aftercare program to follow to ensure the best possible result.
Outcome
You should notice the effects immediately after treatment and you can return to your usual activities right away. It is advisable to plan treatment in advance of social events in case bruising of other reactions occur.
Adverse reactions
Dermal fillers are generally safe, they may cause a few temporary side effects. These tend to go away on their own in about seven days. These adverse events include but are not limited to: an infection, redness, swelling, tenderness bruising of pain and are usually self-limiting.
Common side effects include:
Swelling, redness, bruising, tenderness Less common side effects include: Discoloration, hardening of the skin, lumps and bumps, numbness, visual disturbances and loss of vision, pain.
Downtime
Dermal fillers do not require much of a recovery period, you may want to take a day or two off work just in case you have any kind of reaction.
Avoid the following for 24 hours after your appointment:
Strenuous activity.
Excessive heat or sun exposure for 3 days after treatment.
Turkish bath, sauna for 2-3 weeks.
Nonsteroidal anti-inflammatory drugs (NSAIDs) and aspirin.
Alcoholic beverages.
Avoid applying make – up to the treated area for the next 24 hours after treatment.
It is recommended to apply ice packs to the treated area for several minutes in order to minimize swelling. Your physician will recommend any specific aftercare related to your individual treatment. The patient should be advised not to apply make-up for 24 hours following the injection and to avoid prolonged exposure to UV rays, temperatures below 0°C, saunas and Turkish baths for 2 to 3 weeks following the injection.
Follow up appointment
A follow up appointment is usually offered a month after treatment however we will see you sooner if required.
Frequency of treatments
Several months to 2 years after treatment dermal fillers become degraded following which new treatment can be carried out. The period of time between treatments will vary depending on product used, treatment area(s) and individual differences. Your physician will also have their own preferred timeline, post procedure, where they can assess your treatment results over time to determine the optimal time to ‘top-up’ different treatment areas, for the best results.
MicroTox for Facial Aesthetics
MicroTox is a diluted form of botulinum toxin A (BoNTA) injected into the skin rather than the muscles for cosmetic improvements in skin condition. Several different commercial preparations of BoNTA are available and their use is off label.
☑ MicroTox results in skin improvements via decreased exocrine activity in the skin. They inhibit calcitonin gene-related peptide, increase vasoactive intestinal peptide release and possibly stimulate collagen synthesis. They also decrease keratinocyte activity, cause fibroblast contraction, and reduce the production of sebum.
☑ Very small improvements in the skin can be observed after 1 week, with the full effects visible after 4 weeks. These include reductions in redness, oiliness and pore size, mild skin lifting, and an improvement in overall skin tone. The effects of microToxins wear off within 4 to 6 months, so more treatment is needed to maintain the effects.
☑ Possible side effects include bruising, swelling, redness, or muscle weakness near the injection site, and they can last up to 48 hours. Immunogenicity to microtoxins can lead to a reduced response to further treatments, or even to it no longer working. Using a pure form of microtoxin reduces the likelihood of immunogenicity.
☑ Patient selection is important. Microtoxins are most likely to be effective in patients with thicker skin types. They are not suitable for active rosacea. The microtoxin is administered via a series of microinjections. Patients should avoid touching/cleaning their skin for first 2 hours and have a follow-up assessment after 2 weeks.
